Fees for services Effective July 1, 2011


Adult Health
The Adult Health Program is committed to maintaining the health and improving the quality of life for people 18 years of age and older. Services include physical exams, laboratory testing, Pap smears and patient education.


Breast and Cervical Cancer Program
We assist in finding these cancers early and provide screening to those at increased risk who are not financially able to pay.


Family Planning
The Family Planning Program provides patient education, risk prevention, physical exams, laboratory testing, Pap smears, and methods of birth control. Patients are seen on a sliding scale fee, Medicaid and some insurances.


Care Management for At Risk Children
CMARC is a free and voluntary program to help families improve health outcomes of their children from birth to 5 years of age.


Immunization Program
The goals of the Immunization Program are to prevent or control vaccine preventable communicable diseases. 


Maternal Health Program
The purpose of the Maternal Health Program is to provide comprehensive maternity services to childbearing women who are unable to obtain these services through other sources due to financial limitations. Patients are seen on a sliding scale fee, Medicaid and some insurances are accepted. 


Community Services Team
The Community Services Team is one of the many projects Person County was awarded by a grant through the local Person County Partnership for Children. The Community Servies Team Nurse works with licensed childcare facilities and homes to improve health and safety practices.


Communicable Disease Program
The purpose of the Communicable Disease Program is to prevent and control the spread of diseases.


Sexually Transmitted Infections Services
STI services are available by appointment Monday - Friday from 8:00 a.m.-11:30 a.m. & 1:00 p.m.-3:30 p.m. Some same day appointments are available. Assessment, testing, treatment, and prevention services are available. There is no charge for these services. 


Care Management for High Risk Pregnancies
CMHRP is a care management service provided for pregnant Medicaid recipients who are determined to be at risk for poor health outcomes.


Health Education
Equipping the community with resources necessary to live healthy lifestyles.


Laboratory
The laboratory offers support to programs in the health department with tests which are essential screening and diagnostic tools.


Child Health 
The Person County Health Department's Child Health Clinic Program provides well child care to all children under 21 years of age who receive Medicaid or NC Health Choice or who are self-pay based on a sliding fee scale.

Lead Program
Childhood lead poisoning is one of the most common pediatric health problems in the United States, even though it is entirely preventable. The program provides testing and follow up to children with high lead levels.


PostPartum - Newborn Home Visiting
A Public Health Nurse is available to visit Medicaid eligible mothers and newborns in the home environment.


School Health Program
The purpose of the School Health Program is to assure that children achieve and maintain optimum health, in order to promote maximum physical, emotional and intellectual growth. The Person County Health Department works in collaboration with the Person County Board of Education Nurses.


WIC- Women, Infants, and Children Program
The WIC Program provides supplemental food and nutrition education for women, infants, and children (under five) who are at nutritional risk.
